In the Registration/Ballot Request process:
After entering all of your information into the system, on the final screen, you will be prompted to click a button that says, "Download Form." Click on this button. A PDF file should download within seconds. Depending on your browser, you should be able to click to open the file in Adobe Reader. From there, you can print your form.
If the download process did not work:
There could have been a connection problem during the use of the system that would get in the way of the download. It is a good idea to refresh the browser and try again as a first step.

If neither of these issues solves the problem, and especially if you receive an error message, please submit a report to the Voter Help Desk for assistance. To submit a report, click on the “Submit a question…” button in the bottom right corner of this screen (make sure to scroll to the bottom of the screen).

If you saved your data in a Voter Account, you can also login from a different computer to access your form and try to download it from there. This will help you to identify if the problem is with your device or with our system.
